The form items are stored in a meta field wpuf_form, seems like your form is exceeding it’s capacity.

To debug the issue, I installed two plugin: 1) Debug bar 2) Debug bar console. I can execute any query through that console plugin. That way I deleted the meta key using delete_post_meta() function.
