    I've now disabled all extra authentication. Still the same issue.
    Have I misconfigured something in the form or settings?

    Your main site is http, but the admin panel (wp-admin/admin-ajax.php) is SSL enabled, might that be an issue?

    That's correct. A good point.

    I'll try forcing the WPUF form page to always load over SSL and see if that helps. Is there an option to do that in the plugin?

    No there isn't. You may force SSL everywhere in your site by default.

    Loading the page that contains the form over SSL did it!!
