Error: Duplicate labels for custom fields

Jason I have created a form with a set of radio buttons ("Yes" / "No") and a dropdown list of States ("Alabama", "Alaska", etc.) I also have another custom field (URL) with conditional logic, so that it will display if the radio button is set to "Yes." The problem is that when I save the form, all the values from the State dropdown appear as labels in the set of radio buttons. Any idea what is causing this problem?

Hello Jason,
I can not see any radio field in your form and I submitted a post using your form and it took me to the “thank you” page. I guess you have not set the settings right.

You can also check for theme and plugin conflicts by deactivating all your plugins except User Frontend and switching to a default WordPress theme like – Twenty Fourteen.

Please let me know the results.

Sorry, I had to change the form for the client. Here is a test form I set up: http://www.fastforwardroc.org/test-submit/

For the first set of radio buttons, I added the labels “Yes” and “No”

For the dropdown of States, I added “Alabama”, “Alaska”, and “Hawaii” — and when I saved, the plugin also added the labels from the radio buttons (“Yes” and “No”)

As a test, I created another set of radio buttons with the labels “Yay” and “Nay” — and when I saved, all the previous labels had been added to it.
