Form Fields Deleted After Upgrade

bhriv	I have just updated the plugin and 2 of my WP Front End User Pro forms have been deleted. Both in the frontend and the dashboard these forms are blank. I have one form that is still visible and working - which is very strange. I have tried reinstalling the plugin. I've tried resetting my database to an earlier version with the working Forms, but the Forms are still blank. I have tried starting again with a fresh install of Wordpress and reconnecting the database and it's still blank. I am working in a localhost environment using MAMP. Please help asap! Thanks

bhriv	Can anyone provide assistance with this? It seems that the forms that include the HTML element in the form are the ones that break. Forms that don’t include this HTML element work when I upload my localhost version. Thanks
