    Can you send a screenshot of the checkbox field of the form builder? I want to see how the form was configured in the backend.

    Sorry I forgot that you gave us the site details.

    Anyway, the problem seems like you haven't given any meta key for that checkbox field. Give it a meta key and try it again.
