form wont display after update to 2.3.8

Nayem I have pro version and the reason to post here is my cousin bought plugin for me and i cant post in pro section now i have this problem after i updated pro plugin the form wont show please check this http://yadbedeh.ir/teach/

Please download this file- https://www.dropbox.com/s/96vwtg3eewqv75h/subscription.php?dl=0 and replace with WP user frontend pro/class/subscription.php file. After replacing this file everything will work fine in v2.3.8.
