OK looks like the custom post type that was originally setup for the form to post to changed and that caused the issue. Every new post that I have done through the form appears to be working correctly. Where can I post feature suggestions?

I corrected it by having the user that created the post delete it from their dashboard. Funny thing is it did not show up as a post on the admin side. I will try to recreate the error and let you know if it was just an anomaly or a true error.
